Normally (your first log in):
 - Username = "LastName*" (case sensitive) [ examples ]
 - Password = "CMSA number" (only numerals) 

Multiple Members with the same "LastName"  (if the above does not work, try this) [ examples ]
 - Username = "FirstName plus LastName*" (case sensitive, no spaces)

* MAIDEN NAMES ARE NOT INCLUDED IN "LastName" [ examples ]
